For academic misconduct specifically, Policy 541's decision-maker table (§5.8.1) names the student conduct administrator as the faculty member of the course in which the misconduct occurred, with the appeal decided by the academic dean of the college or school, or a designated department chair. Sanctions in the policy (§4.7.2) range from academic sanctions — "failing grades, reduced grades, and/or redoing academic exercises" — through warning, probation, loss of privileges, suspension, expulsion, and "Revocation or withholding of degree." Policy 541 also states a right of access to the Ombuds Office: students have "the right to access the University's Ombuds Office for consultation and assistance resolving matters of personal and school issues" (§4.2.5.6).

UVU's Student Rights & Accountability student page states the same process in plainer terms, with two provisions worth reading side by side with the policy. It states "Faculty are required to report the incident to the Student Conduct Office," and on appeals: "If you are found responsible for a violation of the Code of Conduct, you may appeal disciplinary action in writing within ten (10) school days of the date you were notified of the outcome." That ten-school-day figure sits beside Policy 541's five-school-day appeal window quoted above; both are reproduced here as published, and which governs any case is not determined on this page. On records, the same page states: "While a student's disciplinary records are not reflected on their UVU transcript, they most often become an issue if the student plans to attend graduate school or law school, or apply for federal employment."
